The At-Risk of Transition Register is a simple Google Sheets tool that helps schools identify and prioritise students who may require additional support when transitioning into a new school. Using a clear scoring system, staff can quickly see which students may be at higher risk and plan support accordingly. This resource is provided as a shared Google Sheet template, not an app, allowing schools to make their own copy and securely manage all student information within their own Google Workspace environment.
