The At-Risk of Transition Register is a simple Google Sheets tool designed to help schools identify students who may require the most support when transitioning into a new school.
The register allows staff to score and prioritise incoming students based on identified needs, helping schools quickly determine which students may be at higher risk during transition and therefore require earlier planning, support, or monitoring.
