The At-Risk of Transition Register is a simple Google Sheets tool designed to help schools identify students who may require the most support when transitioning into a new school.

The register allows staff to score and prioritise incoming students based on identified needs, helping schools quickly determine which students may be at higher risk during transition and therefore require earlier planning, support, or monitoring.

By using a clear scoring system, staff can easily see which students should be prioritised for additional support before the start of the school year.

This resource is provided as a Google Sheets template rather than an application, allowing schools to make their own copy and manage all student information internally within their Google Workspace environment.
